Request for Service – To establish if we can provide you with our services, at first contact we will collect information including but not limited to your name, date of birth, sex, address, phone numbers, email address. We may also request information about your availability, presenting issues/needs, history, brief description on why you’re interested in our services and other details we deem necessary to process your request.

Initial Consultation – At an initial consultation we enquire about your current personal, emotional, social and medical situation, the issues affecting you now, as well as your background and family history. We may also ask about your financial situation to establish payment fees for those who have financial need. This information is necessary for us to decide if we are a good fit and for us to manage and ensure the quality of service we provide for you. If you agree to use our services you will be asked to sign our Terms of Service Agreement and Personal Information Collection Agreement/Informed Consent Agreement to give us permission to process your data, as well as complete a confidential Intake Form.

During Sessions, Workshops, Programs or Events – Any notes taken are anonymized and your identity is protected. Personal data that identifies you is kept securely and separate from our notes to maintain anonymity. We will ask your consent to record sessions for the use of record keeping. Recordings from sessions are confidential and stored securely with password protection. All notes and recordings are the property of Wholeness Wellness Bar and not the service user. We reserve the right to refuse any requests to share information with third parties that are not required by law. 

Payment Data – We collect the data needed to process your payment if you use our online store or engage in any of our services. This may include payment instrument numbers (i.e. credit card numbers), associated security codes, billing address and contact information. All payment data is handled by our payment processer. Please review their privacy policies and contact the payment processer directly for any queries.

Other – Relevant information will be collected for enquiries, feedback, sign ups, marketing, giveaways and events and may include your name, address, email address, contact and other messages to us.

Website – We use Google Analytics to collect anonymous data relating to user behaviour and web traffic statistics. The collection and use of this data by Google Inc. is subject to their own privacy policies.

You agree to use our services and submit information at your own risk and that Wholeness Wellness Bar is not liable regarding unauthorized access. Computer Files: Documents stored on computers will be anonymized or password protected.

Procedural Safeguards – Documents containing personal information will be kept on record for up to one year upon termination of service after which such documents will be shredded.
